These perks are real on many ACA plans for 2026. UnitedHealthcare has tiered gym access, starting with digital for $0 dollars. Blue Cross and Blue Shield of Texas offers a $28 dollar gym program through Blue365. Cigna pays up to $325 dollars in rewards. Ambetter from Superior HealthPlan in Texas pays up to $500. Oscar has simple app rewards.

UnitedHealthcare: One Pass Select
What you get
Digital fitness classes are $0 for ACA members in 2025. That includes thousands of on-demand classes. UnitedHealthcare - in 2026 expect the same plan to continue with even better incentives.
A tiered gym plan. You can use all gyms in your tier and you can change gyms any time. You can think about it as a “use any gym in your tier” setup.
2025 member pricing estimates:
Digital: $0
Classic: $34/mo about 12,000+ gyms
Standard: $69/mo about 14,000+ gyms
Premium: $109/mo about 16,000+ gyms
Elite: $249/mo about 20,000+ gyms, including many premium and boutique clubs
Multiple employer and municipal pages list the $34 / $69 / $109 / $249 ladder and 12k to 20k+ locations.
Cash Rewards Incentives:
Most plans include a $50 gift card incentive when you set up auto pay, choose your PCP and create an online account.
Additionally receive another $100 gift card reward incentive when you schedule and visit your PCP for the first time.
Total cash incentive is $150 - some limitations apply (no alcohol and firearms, etc) but you can use it to pay your bills, groceries, spa, salon, haircut, gas… you get the picture.

What you get

Aetna CVS is leaving the ACA marketplace for 2026
Aetna CVS Health says its ACA Individual and Family plans will end after 2025. If you are on Aetna now, you will need a new plan for 2026. We can help you move to UnitedHealthcare, BCBSTX, Cigna, Oscar, or Ambetter.
