Turning 65 With ACA Insurance? Avoid a Medicare Coverage Gap

Turning 65 while on an ACA Marketplace plan? Learn when Medicare starts, how Marketplace savings change and how to time your transition.

A sixty-fifth birthday usually brings cake, phone calls, and a surprising amount of mail about Medicare. For someone who already has health insurance through the Affordable Care Act Marketplace, it can also bring a dangerous assumption: that one kind of coverage automatically hands off to the other.

It doesn't. Marketplace insurance and Medicare are separate systems, with different enrollment rules and financial assistance. A smooth transition requires paying attention to dates, not just selecting a new plan.

Your Marketplace plan does not automatically stop

HealthCare.gov explains that when Medicare begins, Marketplace coverage does not simply disappear. You generally need to update your Marketplace application and end coverage for the person moving to Medicare. If other family members still need Marketplace coverage, they may remain enrolled if eligible.

That matters for couples with different ages. One spouse might become Medicare-eligible in May while the other still needs an individual health plan for several years. Canceling the entire household's coverage could create an avoidable problem.

The transition should be done person by person, with effective dates verified before any coverage ends.

Medicare's enrollment calendar has its own rules

Many people become eligible to enroll in Medicare around age 65, but the details depend on whether they're already receiving certain benefits and whether they have qualifying employer coverage. Medicare's Initial Enrollment Period generally surrounds the birthday month. Delaying Part B without qualifying coverage can result in a late-enrollment penalty or a gap.

Marketplace coverage is not considered the same thing as current-employment group coverage for Medicare Part B enrollment purposes. Do not assume that paying an ACA premium allows you to delay Medicare indefinitely without consequences.

Plan for the transition several months ahead so you can establish when Part A and Part B coverage will actually take effect.

Our turning-65 guide to Medicare and Medicaid explains the broader coverage choices.

What happens to ACA premium assistance?

Marketplace financial help can change when Medicare eligibility begins. HealthCare.gov states that once someone has Medicare Part A or Medicare Advantage, they generally are not eligible for Marketplace savings for that person. Medicare.gov also warns about subsidies for people who become eligible for premium-free Part A.

This is one reason keeping both systems running without careful coordination can be costly. Someone may continue receiving advance premium credits and later discover those amounts need to be reconciled at tax time.

Do not use a birthday alone as your termination instruction. Confirm the actual Medicare start date and the proper Marketplace end date. For example, if Medicare begins on the first day of a month, Marketplace coverage for that individual may need to end the day before.

Then comes the question of which Medicare coverage

Original Medicare and Medicare Advantage offer different ways to receive Medicare benefits. Costs, provider access, prescriptions, prior authorizations, and out-of-pocket protections vary. A Medicare Advantage plan may have network rules; Original Medicare may be paired with other coverage depending on eligibility and enrollment rights.

The right comparison is personal. Someone who sees a specialist across state lines may prioritize provider access. Someone with an expensive medication should check the drug formulary and applicable pharmacy costs. A frequent traveler should understand how care is handled away from home.

And if the spouse stays on Marketplace coverage, the household income and tax information may still need attention even after one person leaves the plan.

Mark these milestones before the birthday

Start by checking Medicare enrollment eligibility and dates, then review the Marketplace policy's renewal and termination procedure. Confirm that any chosen Medicare coverage is approved and has an effective date. Only then arrange to end the appropriate Marketplace coverage.

The Baylon Agency helps clients in Texas and New Mexico work through these comparisons without assuming everyone's sixty-fifth birthday creates the same enrollment situation.

Frequently asked questions

Does ACA insurance automatically switch to Medicare at 65? No. Medicare enrollment and Marketplace termination are separate actions.

Can my younger spouse stay on the Marketplace? Often yes, if otherwise eligible. Update the application without automatically canceling the whole household.

Can I keep receiving ACA tax credits after Medicare starts? Generally not for the person whose Medicare eligibility or enrollment removes eligibility for those savings; confirm the specific situation promptly.
